This Form has been used by the author for over 25 years to keep her 5 children from being forced to get vaccinated to attend school. These forms work because, it means this parent is a fighter, one whom will make trouble for them (they hate being sued), because they know there is always the possibility of damage. A Wild Card would expose their mistakes, maybe ruin their business-as-usual, and they have enough parents frightened enough to support their campaign.
